Please use a ballpen to fill in the application form with BLOCK letters.

When you submit the application form, please bring together a photo (1.5"x2") of your child and a photocopy of his/her birth certificate.

Kindly provide a 4"x9" return envelope with stamp and your child's name and address printed on it.

Please keep the number on your application form for future reference.

You'll be notified the result of your application 2 weeks after the interview.
